IU1 Lending Library COVID-19 Release from Liability
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Intermediate Unit 1 has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Intermediate Unit 1 can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, IU1 staff, school district staff and students and their families.
I voluntarily seek services provided by Intermediate Unit 1 and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Intermediate Unit 1 harmless from, and waive on behalf of myself, school staff, students, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act on Intermediate Unit 1, or that may otherwise arise in any way in connection with any services received from Intermediate Unit 1. I understand that this release discharges Intermediate Unit 1 from any liability or claim that I, school district staff or students, my heirs, or any personal representatives may have against Intermediate Unit 1 with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Intermediate Unit 1. This liability waiver and release extends to Intermediate Unit 1 together with its directors, officers, administrators, employees, and agents.
IU1 Lending Library Device Sanitation Protocol
Staff and Client Safety
IU1 staff will follow current guidance provided by the Center for Disease Control and PA Dept. of Health.
PPE will be worn at all times with the exception of while they are working from their computer at their designated workspace.
Logistics
Scheduling
Lending requests will continue to be submitted via IU1 Lending Library Website.
Upon request submission, an IU1 staff member will schedule an appropriate pick-up and drop-off date based on availability.
Items will have a six (6) day turnaround time, where items on loan are to remain unused and untouched by students and staff for a minimum of three (3) days prior to pick up or drop off. Once items are returned and in the office they will remain untouched for another three (3) days before the sanitization process and further loans.
All pick-up and drop-off requests must be determined prior to arriving at the IU or prior to school drop off.
No before/after hours pick-ups or drop-offs are permitted.
Lending of multiple kits to stagger borrowing of the same kit will be done whenever possible.
Location
Return
The borrower is responsible for following sanitization guidelines prior to return.
- Items on loan are to remain unused and untouched by students and staff for a minimum of three (3) days prior to pick up or drop off
Same procedures listed above will be followed when containers are returned.
Devices will be stored for a minimum of 72-hours prior to sanitation in a location to be determined.
